MEDICATION: - We do not call in antibiotics on the phone. If your child is sick enough to need an antibiotic, he/she needs to be seen by a physician.
We require that your child be current with their WCC appointments to refill medications
. Please give us at least 24 hr notice for routine medicine refills and 72 hrs for ADD medication refills.
We are unable to call in prescriptions written during office visits. You will be given a signed prescription to take to your preferred pharmacy.

IMMUNIZATIONS: - We require presence of at least one parent/legal guardian when immunizations have to be administered to your child. 

We do not accept any new patients who decline immunizations for their children. The physicians at LLP follow the American Academy of Pediatrics guidelines on immunizations

If you need a copy of your child's immunization, you can request a copy over the phone after sending us a signed medical records form. Please download this form, fill and sign it before faxing over the request:

PRIVACY: - We follow HIPPA guidelines in protecting your privacy

APPOINTMENTS: -We do not have "walk-in" hours. An extra "walk-in" fee may be charged if you do not have an appointment. - Please inform us 24 hrs in advance if you are going to cancel an appointment. If you miss 3 or more appointments without informing us, we have the right to discharge the patient from our practice.

MEDICAL RECORDS: - Our office maintains all medical records electronically. We follow HIPAA guidelines in maintaining privacy of all our medical records. If you need to transfer records in or out of our office, please fill out "release of records" form. Please allow 2 weeks after requesting medical records. We will send your child's medical records to your home address. A fee may be charged if records are requested more than once.
